Job Description:
A Registered Nurse (RN) Case Manager is responsible for
coordinating patient care and ensuring that individuals receive the
appropriate medical services. They assess patients--- needs, create
personalized care plans, and work with healthcare teams to deliver
the best treatment. The RN case manager helps patients navigate the
healthcare system, educates them about their conditions, and
advocates for necessary resources. They also monitor progress,
manage resources, and ensure that care meets quality standards
while managing costs. This role requires strong communication,
organizational skills, and clinical expertise to improve patient
outcomes and overall care quality.
