TravelNurseSource is working with Lancesoft Inc to find a qualified Case Manager RN in San Francisco, California, 94110 Pay Information $3,128 per week About The Position The RN Case Manager is responsible for Care Coordination, Care Transitions, and Discharge Planning throughout the acute care patient experience. Will be responsible for completing timely Initial Assessments, updated notes, and final notes. Will complete referrals to post acute levels of care.
